Confirm Order shows error "Not Found. Please contact the store owner." [Urgent Help Needed]

Hi,
I have 2 portals, one is started from version 4.0 is working fine with the same files. 2nd I have migrated from 3.7 to 4.0 has shows this message "Not Found. Please contact the store owner." when i click Confirm Order.
Console shows following error
http://localhost:15536/RealOnePageCheckout/ConfirmOrder 404 (Not Found)

what can be the reason?

Thanks for your reply. The issue was actually because of my previous data but plugin wasn't showing me the error thrown by exception handler. But its resolved now.
